New Zealand firefighters wishing to register for cover under the NZPFU group income protection policy administered by Protect should complete and return to NZPFU, the two part authority form linked below, inclusive of:

1. Authority and declaration selecting to join the NZPFU Protect income protection policy.

2. Direct debit authority for weekly bank payments to be made to the NZPFU insurance account.

NOTE: If this Authority is signed and received by NZPFU anytime throughout the month of July 2023, your cover will be backdated to 1 July 2023.
